The lawyer representing Antony Waterlow, the man accused of murdering his sister and father, has said he fears for his client's welfare in custody.

Anthony Waterlow was refused bail by Sydney court yesterday over the stabbing deaths of his 68-year-old father Nick Waterlow and his 36-year-old sister Chloe Waterlow in Randwick on November 9.

The 42-year-old appeared briefly at Parramatta Court a day after he was arrested by police following a standoff in bushland near Colo Heights in Sydney's northwest.

His lawyer John Meadley said the 42 year old did not seem to be himself while appearing via video link.

Mr Meadley said Waterlow has 'issues' which he plans to address in court when he re-appears on December 8 on two charges of murder and one count of grievous bodily harm.

A two-year-old girl, who can't be named for legal reasons, also sustained a cut to her throat in the attack.
